FIN (Family Information Notebook)
fin (fin) n. 1 a fish uses its fin to balance, steer, and avoid obstacles as it moves through the water

FIN stands for Family Information Notebook. The notebook is designed to help families coordinate many aspects of their child’s care and health. It can help families and others caring for a child exchange information so the child’s special health care needs can be met appropriately. Families may share copies of any part of their notebook with anyone who works with their child, including family members and friends.

  • FIN makes getting care for a child easier and faster. The information is handy when the family or someone who works with a child needs information quickly.
  • FIN can help medical personnel, doctors, dentists, nurses, therapists, educators, and other caregivers learn about a child's care and prevents the family from having to repeat the same information again and again.
  • FIN can help families with filling out health reports and forms for agencies and services related to their child’s care.
