If your insurance plan requires referral authorization, please obtain a referral from your primary care physician before scheduling the appointment.
What to Bring to Your Appointment
- A copy of your insurance card
- Parking ticket for validation
- Snacks
- Blood glucose log
- Food records
- Blood glucose meter
Please make every effort to keep your scheduled appointment. If you are unable to do so, it may result in a delay in our ability to see your child. If you need to cancel an appointment, please call as soon as you know you are unable to keep the appointment so we can offer your child's appointment to another child.

Vanderbilt Eskind Diabetes Clinic at the Vanderbilt University Medical Center. Located on the 8th floor of Medical Center East-South Tower. Park in the East Garage and take elevators to the 8th floor. Click here for driving directions.
Satellite Physician Office Franklin.
The Monroe Carell Jr. Children's Hospital at Vanderbilt has a satellite clinic in the Franklin area in the Vanderbilt Medical Group Franklin building. Please call the main endocrinology or diabetes phone numbers to inquire about appointment availability at this site. The address of the clinic is:
